New Case Management Team Coming to Our Village

We are delighted to share that effective July 8, 2024, case management services will return to NNV. Case management will be provided by Aging & Amazing a local care management company that provides support, education, and preparation to empower older adults and strengthen the networks that surround them. With their commitment to "Guiding Care, Cultivating Peace of Mind," they join our dedicated team in continuing to support the wellbeing and mission of our Village.

What is Village Case Management? Our Village’s caring volunteers play an important role in helping older neighbors with practical tasks and remaining engaged in the community. However, sometimes our members require support beyond what a volunteer can provide. Case management bridges the gap between the Village’s scope of services and other care options.

How it works. Full members who are experiencing a life transition such as a hospitalization or rehab stay, physical or memory changes, or the death of a loved one will have access to short-term case management support through the Village. If both the member and Village staff believe time limited case management support would be beneficial, the Village will make a referral directly to Aging & Amazing and a case manager will then reach out directly to the member to provide support.

How much support will I receive? The amount of time provided by the Village Case Manager will vary depending on the unique needs of each member. However, most members can expect around 5-10 hours of Village case management services.

What if I need more than short-term help? Full members who need more than short-term help will be referred to appropriate resources, this may include services offered by the Department of Aging and Community Living, private care management, or other options.

Who pays for additional care? The Village Case Manager may recommend and/or coordinate additional services, such as home care, daily money management, decluttering, etc. Members will be responsible for the cost of these additional care services. Whenever possible, the Village Case Manager will help members find affordable options and/or take advantage of benefits available to them.
